Please read and check the box on the left: I acknowledge that the services offered by Boalsburg Yoga & Wellness/Kerri Zelman are not a replacement for qualified medical and/or mental health care. In consideration of the acceptance of participation in classes, workshops or trainings offered by Boalsburg Yoga & Wellness/Kerri Zelman, I waive any claims of injury or damage against Boalsburg Yoga & Wellness/Kerri Zelman. I also acknowledge that my participation in classes, workshops or trainings offered by Boalsburg Yoga & Wellness/Kerri Zelman supports a safe space and learning community to all participants. As such, we expect all participants to engage in respectful conduct at all times. This includes the receiving and sharing of personal information, experiences and advice, both in person and in all social spaces, connected with the program(s). I affirm that I have no medical conditions that I am aware of which would prevent me from taking part in classes or workshops. In addition, I will make the instructor aware of any medical conditions or physical limitations before class. I have read and fully understand and agree to the terms of this Liability Waiver Agreement and completely and unconditionally release Boalsburg Yoga & Wellness Studio/Kerri Zelman from any and all liability for any loss, damage, injury or expense that I may incur as a result of participating in the programs offered by Boalsburg Yoga & Wellness/Kerri Zelman due to any cause whatsoever.
